Q:  I'm attending in-person and I forgot to print out the required forms (Liability Release & Waiver Form and Youth Medical Release Form) .  Where can I find these forms?
A:  www.floridafummwa.org/2021-workshop-forms-for-in-person-attendees.html

Q:  Where do I send my forms?
A:  Liability Release & Waiver Form:  Scan & E-mail to Registrar

Q:  I need to access my registration.  How can I do that?
A:  When you registered, you should have received a confirmation e-mail.  At the bottom of the e-mail, there is a message:  "Would you like to create an account? Creating an account allows you to manage your history and make changes to your information."  Click on the "Set Up An Account Now" link and follow the directions.  This will allow you to access your registration and make changes as needed.  Make sure you reply to the "Welcome" e-mail and bookmark the link that will allow you access to your registration.

Q:  Where can I find the schedule for the week?
A:  Both in-person and virtual schedules can be found at:  www.floridafummwa.org/workshop-2021---from-generation-to-generation.html

Q:  I'm attending in-person.  How do I know what to bring and what time I should show up?
A:  Approximately 2 weeks before the first day of the Workshop, you will receive an e-mail with "What To Bring."  That document will also be posted on our Workshop page on this website.

Q:  I'm attending the Virtual Workshop.  When & how will I get the links for each session?
A:  The week prior to the start of the Workshop, you will receive an e-mail with all of the information needed to access each session.  Also included in this e-mail will be contact information in case you have problems accessing any of the sessions.

 (The Florida Chapter of The Fellowship of United Methodists in Music & Worship Arts is exempt from Federal Income Tax under Section 501(c)(3) of the Internal Revenue Code.  We are further classified as a public charity.  This means that the Chapter can receive contributions that are tax deductible under section 170 of the code.  We are also qualified to receive tax deductible bequests,  transfers, or gifts under section 2055, 2106, or 2522 of the code. )
